District 5100 Host Family Application
In accordance with the regulations of the US Department of State, all host families are required to complete an application.
If you begin the application but do not have all of the necessary information, or for some other reasons need to finish at a later time, there is an option on each page to do so.
The application will require that you provide the following:
- Contact information and email addresses for two personal references; please note that these references may not be your relatives or volunteers with the Rotary Youth Exchange Club or District committees.
- Upload 5 photos of your home – one each of the exterior, the living room or family room, the kitchen, and the bedroom and bathroom that
the student will use and one family photo of all of you. - All family members living in the home, 18 years old and older are required to complete a CBC (Criminal Background Check) and the YPA (Youth Protection Awareness) training and test. Each person will be sent two separate emails with a personal link to these two processes. Please use only your link to the online processes.
- The application also asks for information about your family, community and school. Please complete all pages as best you can.
Once all of this is done, the club will do an in-home interview and orientation that will be documented by a report and form. When all the requirements are fulfilled, you will receive an email that you are an approved host family. This is good for 18 months and then expires. If you have already been an approved host family in the past, we can send you a link to your old application that you can just update. Please let
us know if this is the case.
